As part of the program individuals have an opportunity to reduce their health care premiums by the year 2010. If you are a participant in the Wyoming State Employees’ Group Insurance Health Plan, including community colleges and the University of Wyoming, and retired employees who continue their health insurance through the state plan, you will receive through the mail, a personalized Wellness Insurance Premium Discount form (see sample form below). Please watch for it and do not discard. You will need to return this personalized form in the envelope provided, in order to receive your wellness premium discount.
The requirements of the program are as follows:
To receive a $40 per month wellness insurance premium discount, a total of $480 for plan year 2010, the following three Healthier WY requirements must be completed no later than September 30, 2009.
